When submitting a claim I received a notice to complete a form that asked for every doctor, PA, surgeon, etc that my wife and I have seen for the past 5 plus years.  I think they asked for the past 7 years, i can't remember.  The notice stated that if it was found that a provider was not listed, it could result in a claim denial.  My wife has lupus so she has seen a number of providers in 7 years and I know it would be impossible to name everyone.  I have never had an insurance company to ask for such information.  Every claim that was submitted was denied. Everyone of them.
